It could also mean prostate cancer - but thats pretty extreme. What it most likely indicates is inturnal bleeding of some kind. Other possibilities could include that you have an std, infection, or a rupture in the blood vessals around either the testies or the vas deferance (the thing that regulates semen flow).

Other questions you should be asking yourself -

Does it hurt to pee or ejaculate?

Does your family have a history of prostate issues?

Did this occur only once and has not happened since?

Have you recently been in a fight, car accident or in some instance where you might have gotten damaged to either your kidnies or bladder?

Have you noticed any strange smells or puss?

Any rash or strange bumps?

Other pain or swelling?

Your doctor will have more questions for you, just think about it before you see him. Like others are pointing out, its never a good sign when you see blood come out your penis.
